Transcript and Certificate Request and Release Policy

Transcript and Certificate Requests:  
Any student who has attended the school and completed a program may request a copy of their certificate and/or transcript by contacting the school at admin@clinicalherbalism.com.  Requests will be processed promptly and the document sent electronically or hard copy, as requested. Please convey information to confirm your identity and also information to confirm the dates of attendance. This information will only be shared with individuals who graduated from CSCH and/or entities given explicit permission by the graduate (e.g., schools, government agencies, prospective employers, etc.)

Communications Regarding Loans: 
CSCH does not issue loans and does not administratively manage any educational loans.
 
Official Policy
A. Purpose 
This policy is in response to Colorado HB22-1049. 
 
B. Scope 
This policy applies to all current and former students, excluding foreign students as defined in C.R.S. 23-1-113.5., who have an outstanding debt for tuition, room and board, or other financial aid funds owed to the school and request a transcript, diploma, or certificate. This policy also describes when a student may be subject to a registration hold. 
 
C. Policy 
  1. Request for Certificate and/or Transcripts.Current or former students at the Colorado School of Clinical Herbalism may request and receive a copy of their certificates and/or transcripts by writing to bookkeeping@clinicalherbalism.com.  There is no charge for students who are currently enrolled at the school.  Students who attended the school more than one year prior to the request shall pay a $20 fee for each transcript and/or certificate in order to cover the administrative and mailing costs associated with producing the transcript and/or certificate.  If students need a letter to accompany the transcript/certificate, please indicate in the writing the individual or entity to whom the letter should be addressed and the purpose of the transcript/certificate copies.
  2. Holds on Certificates and/or Transcripts.  Individuals may be subject to a transcript, diploma, or certificate hold when such individual owes certain debts to the School for tuition, fees, or financial aid funds.  Individuals will be subject to such a hold when more than 30 days have passed from the end of a program and a student has not completed all payments defined in the Enrollment Agreement.  Unless an extended payment plan is arranged in writing between the student and the school, any unpaid tuition exceeding $500 will be turned over to a collection agency.  If an individual is subject to such a hold, their transcripts, diplomas, or certificates will not be released unless an exemption applies as outlined in section 2 of this policy.
    1. Exemptions on holds are granted for individuals who can demonstrate that the transcript/diploma/certificate request is required for one of the following reasons:
      1. Job application
      2. Transferring to another postsecondary institution
      3. Applying for state, federal, or institutional financial aid
      4. Pursuit of opportunities in the military or national guard
      5. Pursuit of other postsecondary opportunities
    2. Process and Procedure for Exemptions: 
Upon submission of a transcript/diploma/certificate request and documentation to verify an exemption, the School will review the request and make a determination regarding whether or not an exemption exists, as outlined in section 2, above.  If it is determined that the individual has a valid exemption, the School will release the requested transcript/diploma/certificate.  If the School determines that the individual does not meet the exemption criteria, the School will provide a written explanation of the denial of the request within seven business days.
    3. Registration Holds: Unless otherwise arranged in writing between the school and the student, individuals will not be able to register or enroll for additional upcoming courses/classes if the unpaid debt is not paid within 15 days of the start of the next program that the individual would like to attend.
  3.  Individuals have the option to establish a payment plan for an outstanding debt. An individual who wishes to establish a payment plan for a debt owed to the School should contact either one of the school directors or email admin@clinicalherbalism.com

  4. Complaints pertaining to HB22-1049 may be submitted to the Colorado Student Loan Ombudsperson via email to CSLSA@coag.gov.
